Death Certificates

Death certificates without cause are available to anyone. However, cause of death is available only to immediate family members, with appropriate photo ID. Lawyers representing the immediate family, banks, executors of estates, and insurance companies can also obtain copies with cause with appropriate documentation.

Death certificates are only available for deaths occurring in Okaloosa County from January 2000 until present. The cost per copy is $10.00.

  • Require completed application and payment by check, money order, or credit card
  • With cause requests require notarized copy of valid photo ID
  • Credit card transactions by mail require an additional $5.00 processing fee
  • Federal Express mail service is available with credit card purchases for an additional $16.00 fee

  • Please include a self-addressed - stamped envelope

Medical Amendments
  • To amend the medical certification or cause of death section on the death certificate requires the certifying physician or medical examiner to complete and file the notarized Affidavit of Amendment to Medical Certification of Death, DH 434A (2005 Version) or Affidavit of Amendment to Medical Certification of Death, DH 434 (2004 and prior). This includes not only the cause of death and manner of death, but also"...the date of death, hour or time of death or the place of death other than street address..."
  • There is no amendment fee associated with a medical amendment; however, if a certification of the amended death record is desired, a fee of $5.00 for the first copy and $4.00 for each subsequent copy ordered at the same time is required.
